Description

TJ Samson Community Hospital (TJSCH) is looking for a highly skilled Clinical Staff Pharmacist to join the Inpatient Pharmacy team. As the flagship institution of TJ Regional Health, TJSCH is a 196-bed teaching community hospital located in Glasgow, Kentucky. In addition to the main campus, TJ Regional Health includes TJ Health Columbia, TJ Health Pavilion, TJ Health Partners, and a number of outpatient clinics. The Department of Pharmacy includes two-acute care inpatient pharmacies, two-outpatient pharmacies, and one-oncology infusion pharmacy.

Opportunities for Clinical Pharmacists involve direct patient care, including interdisciplinary rounds, transitions of care, code response, consult services, antimicrobial stewardship, and hospital quality measures. As a pharmacy student rotation site, education and precepting opportunities are available.

Applicants must be a graduate of an ACPE-accredited school of pharmacy and be a licensed (license-eligible) pharmacist in Kentucky. PharmD preferred. PGY1 Residency and/or previous clinical experience in a hospital pharmacy setting are preferred. Board of Pharmacy Specialties or equivalent certification preferred.

Job Type: Full-time (40 hours/week)

Required education:  Bachelor's in Pharmacy; Doctor of Pharmacy preferred. PGY1 residency preferred. Board of Pharmacy Specialties or equivalent certification preferred.

Required licenses or certifications: Pharmacist (Kentucky) and graduate of ACPE-accredited College of Pharmacy

JOB SUMMARY

  • Reports to Manager, Pharmacy Services.
  • Compounds and dispenses medication pursuant to a physician order.
  • Monitors drug therapy for appropriateness of drug, dose, route, and duration of therapy.
  • Screens for drug interactions, incompatibilities and contraindications.
  • Recognizes potential adverse drug effects and intervenes as appropriate.
  • Provides drug information to physicians, nurses, allied health professionals, and patients.
  • Participates in interdisciplinary team conferences and other patient-care activities.
  • Cares for all age group patients (birth and above).


Requirements

JOB REQUIREMENTS

Minimum Education

  • B.S. or Pharm. D. degree from an accredited College of Pharmacy.
  • Must provide evidence of meeting state mandated continuing education requirements upon request.

Minimum Work Experience

  • Previous hospital pharmacy experience is highly desirable.
  • Previous pharmacy computer experience desirable.

Required Licenses

  • [Kentucky, United States] Pharmacists
  • Licensed as a Registered Pharmacist by the Kentucky State Board of Pharmacy (or must be Kentucky licensed within 90 days of employment).
